Please visit http://www.ipkall.com.  Sign up for a number and, once you receive it, log back in and update the details so that:

* the SIP phone number is 1 + the rest of the number
* it's delivered over SIP
* it's delivered to sip.softivr.com

Once finished, the configuration screen should look like this:
